I authorize Apex Wireless to add the selected $12 per line AKKO Protection plan to my wireless account for the mobile number provided. I understand this is a recurring monthly service that will be billed through my Evergreen account until canceled. I certify that I am the account holder or an authorized user on this Evergreen account and that the information I have provided is accurate. I understand that all claims, coverage, and protection services are managed directly by AKKO, not Apex Wireless. I also understand that I must complete my AKKO registration by following the activation email sent by AKKO after my enrollment has been processed.
